Salon Bales, PC (“we,” “our,” or “the practice”), using the practice name SALON BALES under Medical Board of California Fictitious Name Permit FNP 561157, is a covered entity. Clinical records are maintained in our clinical systems. This marketing website is not a designated record set.
Our duties
We are required by law to maintain the privacy of protected health information (PHI), to provide you with this notice, and to notify you of a breach of unsecured PHI as required by law. We must follow the terms of this notice until it is replaced.
Uses and disclosures without written authorization
- Treatment. We may share PHI with clinicians, Qualiphy for good-faith exams, and independent compounding pharmacy partners (Strive Pharmacy and Empower Pharmacy) as needed to provide care. These are independent compounding pharmacy partners. They fill prescriptions when a clinician of Salon Bales, PC determines it is clinically appropriate. Salon Bales, PC is not a pharmacy and does not operate a retail pharmacy. Third-party accreditation or certification marks belonging to these pharmacies are not displayed on this site.
- Payment. We may use PHI to bill and collect for services, including membership packages.
- Health care operations. Quality review, training, licensing, and practice management.
- As required by law, for public health, health oversight, judicial processes, law enforcement, and to avert a serious threat to health or safety, as permitted by HIPAA and California law.
Uses that require authorization
Marketing that uses PHI, sale of PHI, and most uses of psychotherapy notes (if any) require your written authorization. You may revoke an authorization in writing, except to the extent we have already relied on it.
Payment processors
Payments are processed securely by Stripe. Stripe, Inc. is the payment processor and merchant services provider for card payments and related billing. This marketing website does not store full card numbers.
Telehealth
Telehealth is currently limited to patients located in California. Qualiphy is our named partner for good-faith exams. Information you submit to Qualiphy is received by Qualiphy and our clinicians for treatment; it is not collected through the public marketing contact form.
Your rights
- Request restrictions on certain uses (we may not be required to agree, except as the law requires for items paid in full out of pocket).
- Request confidential communications at an alternative address or by alternative means.
- Inspect and obtain a copy of your designated record set, with limited exceptions.
- Request an amendment of PHI you believe is incorrect or incomplete.
- Receive an accounting of certain disclosures.
- Receive a paper copy of this notice.
- File a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate for a complaint.
