The contact form collects name, email, phone, pathway interest, and a short message. We instruct you not to send PHI. Messages that appear to contain medical detail may be rejected so they are not stored on this marketing surface.
Processors
This marketing website uses Stripe for payments and Qualiphy for clinical exams when you are referred. It does not collect protected health information or run the full medical intake.
Stripe (payments). Payments are processed securely by Stripe. Stripe, Inc. is the payment processor and merchant services provider for card payments and related billing. This marketing website does not store full card numbers.
Qualiphy (clinical exam when referred). Qualiphy is an independent telehealth and good-faith examination (GFE) platform used for clinical evaluation and prescription workflows when clinically appropriate. This marketing website does not collect protected health information and does not run the full medical intake. After a consult or intake handoff, the patient completes medical clearance in Qualiphy’s secure environment. Clinical exams are completed through our partner Qualiphy. Every patient completes medical clearance with a licensed clinician before treatment or medication. Packages and memberships are not a guarantee of medication. Medical clearance may result in a decline of treatment or an alternative care plan.
We use hosting, DNS, and security logs as needed to operate the site. We do not sell personal information.
